Ascend Behavioral Health Services is an outpatient behavioral and mental health organization accredited by The Joint Commission (TJC). Ascend's evidence-based outpatient services includes Skills Training and Development, Targeted Case Management, Medication Management, Crisis Intervention, and Psychosocial Rehabilitative Services (adults only).

Skills Training and Support

This service aims to minimize the negative effects of the symptoms, emotional disturbances or associated
environmental stressors which interfere with the client’s daily living.

Targeted Case Management

This service consists of a specific recovery/treatment plan to help an individual obtain needed services and supports that links the individual with medical, social/behavioral, and educational providers and other providers that provide needed services to address identified needs and achieve goals in the treatment plan.

Psychosocial Rehabilitation Services

Includes independent living services to assist the individual in acquiring fundamental functioning skills needed to reside in the community and avoid more restrictive levels of treatment, to reduce behaviors or symptoms that prevent successful functioning their environment of choice. Service includes symptom management, personal hygiene, nutrition, food preparation, exercise, money management, and community integration activities.

Crisis Intervention Services

Crisis intervention services are intensive, community-based, one to one service provided to an individual who requires services to control acute symptoms that place the individual at immediate risk of hospitalization, incarceration, or placement in a more restrictive treatment setting.

Medication Management

This level of care(LOC) is intended to meet the needs of the individual who has an identified treatment need for medication management.  The purpose of this LOC is to maintain stability and utilize the youth’s and/or caregiver’s natural supports and identified strengths to help them transition to community-based providers and resources.
